Call for Nominations - Quality & Innovation Awards

Application deadline: Friday December 13, 2019

Nomination Process

To nominate programs or initiatives for either a Quality or Innovation award, send a short email to Quality and Innovation Awards by Friday, November 8, 2019. Include the names of the initiative, team member(s) and region.

Note: You are eligible to submit an application even if an expression of interest (nomination) has not been submitted.

Application Process

Step 1: Download the relevant application form.

Note: An initiative cannot be eligible for both categories. Please carefully review the criteria outlined in each application form to select the appropriate stream for your submission.

Step 2: Complete the form.

You will need to provide the following:

  • Details about the initiative and contact information
  • Outcomes of the initiative
  • Supporting documents, if any (no more than 5 pages)

Step 3: Submit the application.

Submit the completed form and any supporting documents by email to Quality and Innovation Awards.

Application deadline: Friday, December 13, 2019.

What happens next

After you submit the form, you will receive a confirmation email within 24 hours.

A Quality and Innovation Award Review Committee will be assembled to review applications.

Award recipients will be notified on May 7, 2020.
