Call for Nominations

Rewarding Quality and Innovation within Cancer Care

Application deadline: Friday December 15th, 2017

The Quality and Innovation Awards recognize significant contributions to quality and innovation in the delivery of cancer care and was established to encourage and reward the development of quality and innovative initiatives aimed at enhancing and improving the performance of cancer care in Ontario. The Awards ceremony is hosted by the Cancer Quality Council of Ontario (CQCO) in partnership with Cancer Care Ontario (CCO). We welcome applications from any institution/practice/organization providing cancer prevention activities and/or care for individuals being treated or living with cancer across Ontario.

CQCO’s framework for defining and measuring quality includes the following dimensions and outcomes:

  • Increases safety
  • Improves effectiveness
  • Improves accessibility
  • Improves responsiveness to patients/families and person-centredness
  • Addresses an equity imbalance
  • Improves integration and/or transitions of care
  • Increases efficiency

Innovation, in the context of the Quality and Innovation Awards, refers to new and unique processes, products, or organizations which are bold, experimental and far-reaching. The goal of innovation should be an improvement in the process, organization or product.

Application Process

Persons wishing to nominate programs or initiatives for either a Quality or Innovation award are encouraged to submit an expression of interest in a short email which includes the name of the initiative, team members, and region by Friday November 3, 2017. However, this does not preclude you from submitting the application. The deadline for the application is Friday December 15, 2017.

A Quality and Innovation Award Review Committee will be assembled to review applications. Please note that an initiative cannot be nominated for both categories. Applicants are advised to carefully review the criteria outlined on each nomination form in order to select the appropriate stream under which the application should be submitted.
